Mutants and retards can come from anywhere. A lot of European noble stock is in inbred to this day and has been purposefully inbred for generations; due to having pretty good genes it took a very long for those small negatives to be compounded to the point where they became issues.
The NPC statement of 'inbreeding bad' is a very simplistic view. A lot of people wonder how humanity could have started off, either through evolution or through Adam and Eve, all because 'inbreeding bad' and there weren't enough people around not to engage in the practice; the answer is that those humans didn't have negative mutations that would be compounded in the same way that many inbred people today might.
