Well that's the point being made here. The NAP is a great guideline in small, homogenous communities with common values and purpose. That's why you can't just implement libertarianism on the level of the nation state. You'll essentially have small communities with limited territory with whom people will sign literal social contracts, based on which one is the best fit for them and their family.

I would argue the only real requirement for Hoppe's "private law society" is for secession to be acceptable at any level (state, city, town, person).
Unfortunately the modern democratic world has decided that secession is bad and will use force to prevent it.
Precisely the problem. Any separation will be a forced separation with many struggles. Furthermore, maintaining exclusivity and freedom of association within those communities will likely involve Hoppe's infamous "forced removals" which, while they may not technically be violations of the NAP, will certainly be perceived as such.
The Middle Ages actually used private law far more than is used now. Most people did not defer to the government/monarchy for matters of law and instead relied on private arbitrators and local community judges, clerics etc. In fact when the monarchy wanted to use force, most people just saw these wars as private affairs between monarchs to attain more resources, not something they were all involved in.
Read Hoppe's essay on The Libertarian Quest for a Grand Historical Narrative. As far as the NAP/natural rights are concerned, humanity has actually significantly regressed.
I skipped the libertarian phase. Went from unwittingly Marxist liberal to moderate to right leaning to crypto-traditionalist apprentice.
It took YEARS of research before I started to drift to the middle; liberal programming is so insidious. It's so pervasive too, if you're raised in it then it will seem to make sense until you try to "know everything" and their logic falls apart. But if you only question it half-assedly then you NEVER escape.
Once I was moderate I was only moderate for a few years before accelerating right. On the political graph I'm just right of absolute center, in the "Progressive" block (what George Bush pretended to be).
Everything is so sickening and stupid though. I like Trump but the other politicians are all garbage and I'm only tolerating this shit because we're promised disclosure and military intervention of government. About damn time... Don't even know if I can even trust the Patriots either, but what's left?
If nothing else I'm trying to remove myself from the state. It'll take a decade or more just to get a decent rural house. They entrench us so deeply that most of us never escape the spiritual ghettos called "cities".
