Doctor said it's not STD and most likely something is with my pancreas or maybe parasites. But he doesn't know for sure. He said to use antibiotics at first, but it didn't help at all, nothing changed.

No pain at all. It's not even itching.
It also don't progress at all. Those pimples remain as they are for months. I tried to remove some of them using my nails on fingers, it removes easily but there's lot's of blood after I do it.
Those on pic, that are more red than other are the ones I damaged.

Do you wear your boxers on that area? Could be an allergic reaction to clothing, I'm assuming you ruled out ingrown hairs. Either way trust your doctor, keep the area well ventilated and clean it regularly.

Try to leave some and don't poke them, maybe they will go away eventually. Remove sugary or fatty foods from diet for few weeks. Or just shave your hair or wash yourself more than usual. Experiment and see how it goes.

Just toss some athletes foot powder on it for a week or two and it'll go away. I've checked tinea, it doesn't look like that. Tinea has red circleshaped stains on skin, while I just have pimples. It looks very closely to herpes zoster, but it can't be it.
Also, even if you continue to imply, that this thing is because of lack of hygiene, you should consider that I had a month of antibiotics treatment. If it was bacteria it would be dead. Viruses don't transfer because of dead skin cells or clogged pores..
Jock itch is a tinea, which is to say it's fungal. It can live under your skin and remain dormant for ages until the right conditions make it reappear (moist and warm, which is why it appears under waistbands etc)
