Every night, at a set time, the machine goes down for cleaning which can take about 4 hours. If you are there late at night, the machine has a good chance of being down for cleaning. It heats the cream up to over 100° to keep it clean.
No, McDonalds is one of the most sanitary fast food places. Corporate has their shit down and makes sure each location is meeting it's high standards whether or not it's corporate or franchise.

They have ketchup, BBQ, sweet chilli, even sweet and sour, but never mayo. I always used to pop in to grab a snack after work, I would hear someone ask for mayo at least once a week.
