On the PS2 the buttons were sensitive to how deep you push them in. I think they retained this on PS3? I'm not sure. Basically if you push the button gently, you hold the enemy and threaten them. If you push hard, you slice their throat. This is explained in the manual. And there must be some kinda codec call about it too? I don't quite remember.
Peace Walker and MGSV did CQC the best.
When i played MGS 1, 2 and 3 i never touched the tranquilizer weapons, on the tanker chapter i looked to slay them most.

How does CQC break the game? It was just another option.
You can up up to enemies even when they're shooting you and instantly knock them up by pressing Up +Circle.
