I did a playthrough on hard recently. Works really well and practically bug free compared to the original state of the game. Difficulty can sitll be "bullshit" but that's just low level DnD for you. It gets better once you get past chapter 1 and start getting better spells/feats.
The random manticores and elder elementals within the first few hours out of fucking nowhere as random encounters are gone though, so yeah, it's kinda fixed.
Usually I start on Challenging, then switch to hard after oleg's, then unfair in act 2

Unironically git gud. There are spells that make your party completely immune to anything the wild hunt does other than damage.
They did fix shit like those owlbears that had the literal stats of a tarasque early on in the game, but I found the game to plummet as soon as you started the whole ending chapter (house beyond time and the following events). Couldn't wait for that shit to be over, even tho I enjoyed the rest of the game

I did beat it too retard. Doesn't mean it wasn't broken as fuck and absolutely ridiculous difficulty spike. The last boss was easier than a random room

They hit like trucks but they are also susceptible to all kinds of CC, there's no reason not to just fuck them over with Sirocco and BRAPcloud even though they could hit like crazy. The only hard part was that they targeted touch AC, so depending on your build, your tank might have been somewhat worthless.
Aside from mass dispell from monarch and realizing how wild hunt gaze works, nothing broken combat difficulty wise. Sinking on floors, flags not triggering and stupid fog puzzles were more annoying.
Regardless, considering the mass dispel for monarchs was taken out since december 2018, the actual mob composition in the hosue has changed, there are even more ways to protect against wild hunt gaze and they have upped xp gain so you are entering the last dungeon at least a level or two higher than you used to, whatever filtered you in the first two months of the game is not there anymore
