Unironically how would you do it?
Would you make it only appear when nude, or would pants textures consider it? Would it have physics, how much? If you want nudity to be taken seriously, how do you not making it comical by having it flop around or clip into the model. How do you consider the variation of penis shapes and sizes, does it become part of character design or do you make stock settings for every character?
Dante's Inferno has a comically large one for Lucifer, Rockstar has some for random NPCs, Conan has character generated ones that are basically a meme. No More Heroes has one for a child character but he's covered in blood so its obscured.
From a character design standpoint it's probably a mercy that nobody actually expects penises in games, they would be a nightmare to implement.
They probably drew it in the actual concept art but removed it for the art book.
I've seen characters in the exact same sort of pose for Japanese concept art where they do draw the penis even though you don't see it.
