Lack of testing. I think it is likely that hundreds of thousands to millions of Americans have already caught it, thought it was the flu and have already recovered from it, based on how many flights from Wuhan were allowed to come into the country before travel restrictions were imposed and based upon the amount of time that has elapsed since all those flights from Wuhan came into the country.
The real scary things are the unconfirmed shit like antibody dependent enhancement, reinfection/recrudescence rate, etc.
