There will always those who want to err on the safe side and will say additional testing is warranted. For example, Canada is currently testing at around 35x the confirmed infection rate.

Sweden, by comparison, has a number of tests similar to Canada, but a much higher infection rate, so Sweden is only testing at 7x the infection rate, less than Japan.

So, in short, these numbers don't tell the entire story.

Japan to date has done about 30k tests, but 15k of those tests are on actual people, since most of them did re-tests. It's not like Japan doesn't have enough tests. They have 300k+ tests, but are only using 10% of them.

From what I can see from news sources and even from Abe-hating immigrant blogs, the Japanese hospital is not collapsing like it is in the West. So either there is no large outbreak, or people are staying at home and quietly recovering as if it were a flu.

It's a discipline problem. Even if it doesn't kill you, some of the symptoms can be fairly severe and prevent you from working. It is also more contagious than a flu, therefore has the potential to take more people out of work.

The right thing to do, for the economy, is to not shut things down, and the people who get sick self quarantine. It has been shown that transmission risk is very low if one is not showing symptoms, so this should be possible.

Unfortunately, westerners are stupid. They'd much rather think of themselves and keep working for money or going out to party if they are only showing mild symptoms, infecting everyone else.

So the only recourse is the government shutting everything down and forcing people to stay home.

Allmost all Japanese doctors diagnose pneumonia patients not only by their chief complaint but also by auscultation, blood tests, X-ray, CT, and so on as your doctors usually do in your country.
PCRtesting only with chief complaint and fever will make test number increased but it makes positive predictive value of PCR decreased.
They think keeping positive predictive value high and treating patients having severe symptoms is the most important in this phase now, and I agree.

Japanese people and streets are actually clean, hand-washing and social distancing is literally the norm there, and people can afford to stay home if they need to. Even if Japan makes PCR test numbers increased, the positive Covid-19 rate is still low. It means this strategy is suitable to keep the positive predictive value high and the negative predictive value low in order to find out the patients having SARS-CoV-2 precisely in Japanese case.
Otherwise, In countries where the positive Covid-19 rate is higher, they should test more as they do.
