Any drug acts as a balance on desired effect vs undesirable effects. There's limits also to how a drug is processed or changed by the body. If you take more than you can metabolise or if it's something that readily metabolised you will increase the levels of compound in the body beyond what the body can cope with. Cellular function is finely balanced and something that throws that balance, like introducing more than can be cleared or chemically altered, will fuck up the cellular environment. That will lead to cells losing function, dying, excreting other compounds, activation of misplaced cellular pathways etc. If cells don't function then the organs and tissues they form will lose function. It's not hard to imagine then how an organ losing function or malfunctioning will have catastrophic consequences in your body. This would lead to really distressing painful or dangerous physical experience.
Paracetamol will fuck your liver. Your body will flood with toxins and that will cause a physical experience that will not equate with pain relief.

but its acetaminophen, not paracetamol...
