The dream world isn't inside Sin. It's an actual physical place out at sea. Dream Zanarkand is a massive, city-sized aeon.

Tidus was an Aeon. The entirety of Tidus' Zanarkand was a giant Summoning created by the Gagazet Faythe. This is literally stated in the game.

I know it's part of the shitty twist but if Zankarland is "real" in a sense that's in a different place from the ruins why does Sin appear as a big ball of water that Tidus has to enter to "escape"?

Sin is literally Jecht and he came there for the express purpose of getting Tidus out, with Auron's help, so Tidus can kill him. The water was just a way to obscure what he looks like to make it more dramatic.

Because the dream world existed in the same physical space as the real world. Tidus didn't travel into the future, his Zanarkand was a dream that existed in Spira's present. Sin just limited technology so much for the majority of Spira so they could never discover it. Even if they could reach it, Sin would kill anyone who got close. Jecht was somehow able to wander to Spira, and after becoming Sin he returned to take Tidus there.
