If the world is persistent and you leave a lootable body behind then NPC's will be able to loot the body.

I remember this happening in NV with boone, he picked the weapons and ammo from dead enemies and use them himself, i guess it works the same way now except with your character

It was intentional in the game. If there are two super mutants one with a bat and one with a machine gun. If you kill the gun one, the other would run over and pick it up. Tbis was intentional and has been in the game since at least fallout 4. The issue was that when the player dies they also leave a corpse but there was no flag in code to prevent AI from touching its contents.

It's not 'accidental', they're just programmed to take weapons where they can find it. From dead bodies or dropped or in chests etc. The bug is they can loot your shit and prevent you from ever getting them back.

You have a point. Up until now their games have been single player games so there was no real point to prevent an enemy from picking up your weapon when you die.
Enemies picking up better weapons has been around since Oblivion. And I believe it has been since Skyrim that they are also able to disarm you and pick up your weapon.
