Yeah, I think it's implied he wears the helmet in the game but the cover art of him with it off seemed to fool people into thinking otherwise.
Either that or it was lost after the cascade thing, since radiation and other hazards effected him.

I thought that too but in the dlc’s where you briefly see him, he’s helmetless. You see it at the part where you’re in the vent as he’s dragged along by the two guys who throw him in the trash compactor, and the part where he jumps in to the xen portal

consider you never actually see yourself put it on... you just touch the suit and magically you're wearing it, I suspend my disbelief and assume we miss the scene of us changing, and somewhere the helmet comes into play. Maybe a compartment behind the suit, the ceiling above, or somebody brings it in.
