It depends on the context.
Some bosses, like Sif, come at you with their all from the get-go, so the more you beat them up, the more wrecked their bodies get. You can also see this in dragon bosses whose tails you can cut off.
Others begin the fight underestimating you, so they initially fight with a portion of their true power, only revealing it to you after you've "proven" to them you might actually be a threat. Guys like Fume Knight and the Dancer of the Boreal Valley only use one weapon until you're persistent enough for them to use their second.
Gael, in particular, however, is of a third type, where his physiology changes during the fight. Of course, mechanically it depends on his health, but in-narrative it was probably only a matter of time of being exposed to another human combatant that would cause him to revert from a soul-devouring beast-man to a more refined fighter. The fact that most of the time, form changes are contextual exemplifies the quality and conveyance of the Souls games.
Most of what he does is all show. When he starts flailing around just gtfo and watch him swing at the air, then go back in. Rolling isn't always needed.
Not him, but for which of the 3 bosses? For Vordt, literally just dodge/block his 3 charges and then run up under him while he starts up his breath attack, keep hitting him under his belly, repeat the process until he's dead. His 2nd phase is easier than the first.

3 really overused the "boss powers up as you fight them" mechanic
It's exciting for endgame bosses like Orphan of Kos and Soul of Cinder, and the occasional boss that throws you for a loop like the Curse-Rotted Greatwood.
But the fact that every single boss in 3 has an anime powerup completely takes away from making certain bosses more unique and memorable.

Oh lmao that's fair. The trick to him is to be patient with his jump-smash attacks. When he releases the souls in his final phase he will jump towards you like 3 times in a row, you might be tempted to go for a hit after one of those attacks, don't, they hit like a truck. Best you just try to dodge his jump attacks and wait until they're done because it's difficult to tell if he's going to follow it up with another one if you haven't fought him enough times. Then wait for the normal sword-swinging attacks like those he had in phase 2, they can be punished in the same way.
The Bloodborne of Dark Souls 3: The Fire Fades Edition?
Just dodge and R1 like every other enemy and boss in the game.
Again it's contextual though. I'm pretty sure Gundyr is the only boss infected with the Pus of Man which causes the big mess to come out - and even then, he turns into a huge glass cannon. Sure, he looks scary but you can stunlock him with Fireball (the very first Pyromancy).

In-narrative, the Soul of Cinder is merely cycling BACKWARDS over all the souls who kindled the flame. Gwyn is the "final form" but it's only because he was the first to sacrifice himself to it.
The Crystal Sage, the Abyss Watchers, Aldrich, Yhorm, and the princes merely underestimate you before deciding to bring their full power to bear.
There are more 3 phase bosses in Bloodborne than DaS3
Bloodborne was smarter about it. Not every single boss in Bloodborne has additional phasing, and some of the ones that do simply gain a buff, like the Blood-Starved Beast.
DS3 copied plenty of things BB did.
You know how the Orphan of Kos doesn't attack you until you get closer, and then he slowly walks towards you until he charges you, something he always does instead of any other attack?
Some major boss in DS3 performs this same action. Abyss Watchers and Slave Knight, to name them
Once you're aware of it you can pull off some neat tricks, especially in cooperative play. Seems like a bad flaw to give to a boss just because BB did it first.
Funny thing is that DaS2 did this shit too, but the bosses that had it set it up better, despite being worse fights in the end.

The reason Gael's second phase changes and he starts fighting upright is because he finally goes Hollow, and so you're literally fighting against the Dark Soul as the final boss of the series. At first he's bestial and feral after untold millenia of killing and gathering the pieces of the dark soul together, when he realizes who you are and that you've come to claim the Soul and bring it back to the painter, he's finally able to let go and hollows now that his duty is complete.
This is why the Hollowslayer Greatsword starts doing more damage to him once the second phase starts. He left all those messages guiding you because he knew once the Dark Soul was fully assembled, he'd need you to be the one to bring it back
