The game plays out like normal, except that any time a character is about to do something that didn't happen in the original, they get attacked by time ghosts who are trying to keep the plot intact. Eg, Cloud attacks Reno in the church with intent to kill and is swarmed by time ghosts to prevent it.
Sephiroth is Sephiroth from the future or another dimension, and he read a copy of the script for the original FFVII. He's trying to change the events of the game to lead to an outcome where he wins. The party gets tricked into thinking the time ghosts are the real villains and are shown visions of Meteor and Aerith's death. The time ghosts are defeated, Sephiroth escapes, Biggs and Wedge survive, Zack survives due to time magic.
