I hate the Lady Of Pain because she doesn't feel like she fits the role of a "Multiverse Guardian." She's just way too specific. A pain entity in charge of protecting a hub to universes? She's like a square peg in a round hole.
She feels more like, y'know, a deity of pain or mazes or contradictions.

Because she's much more of a plot device than a character. Having characters that are omnipotent in their settings interacting leads to powerman-esque crap. She's there to throw a wrench in that, and more questions that might be raised about the worldbuilding.
It's a tool for the DM to create interesting scenarios, and help them avoid powerlevel discussions about gods

She's not a Diety though. She doesn't have a portfolio to match up. No one knows for sure what she even is. The specificity is part of the appeal both for her and for the cosmology of the great wheel overall. There's no reason sigil specifically should be a torus sitting above an impossibly tall spire. It just is, and that's interesting. I mean they could have had Aoskar the god of portals be in her position if they wanted obvious thematics. But they didn't. And in fact they had Lady kill him contemptuously easily. And that's cool.
