It's Midgar only. It's about 40 hours and there are sidequests that are really MMO like - gather this, kill that - I never did those. They added a TON of backstory for everyone and a lot of additional actual story. New characters and systems as well. You can level weapons up with you and such. If you liked FFVII you'll like this.

That was without doing more than maybe 2 of the sidequests. Once I realized the items they rewarded could be found in overworld chests or bought in stores I quit doing them. There are a few 10+ minute cutscenes that lengthened my time I'm sure and I also did a lot of exploring.
