If a spell or artifact has "On Flow" next to an effect, it gets that extra effect when you have flow active.
Any spell that says Gain x Flow gives you that much.
But it goes down for every spell you cast.

I mean it was totally obvious what with the random nature of affixes added to spells but still, I had to be sure, for posterity. I don't really like the upgrade system in this game myself, you can roll an option like "Gain 1 Flow" (when you don't use any Flow cards in your deck) or "Gain 10 Shield" which is not a lot unless you roll it on all of your spells.

There are small ways, like summon a structure then cast link on it so she kills herself with her own attack.
Or if you learn the timing you can dodge it with spells that teleport/step slash.
But yeah it's meant to do that if you go for a neutral ending
