It's just that trying to mirror the pre-war government is only gonna lead to a repeat of history all over again when humanity is restored back to civilization. And then the other nations will threaten to nuke them, and they'll all drop nukes at the same time, etc etc to infinity.

The NCR is the embodiment of scope creep. They are objectively trying to be good, but they also have some very deep shortcomings, and while they solve problems, they also create them. They're simply too ambitious to reasonably maintain their original vision. It's already eroding from FO2 to NV, just give it another 25-50 years and see where it's at then

The nuke we see in Fallout 4 creates a mushroom cloud. Only ground detonations cause such an occurrences, and for the purpose of maximizing radiation, while ICBMs detonate midair to increase destruction and minimize radiation.

The NCR really is not that big. The republic was only about 100,000 people mostly in southern California and it's expansion is not really elaborated on. Redding, vault city, and Reno are all cities that can join the republic and it was strictly stated to have positive effects on the republic. The NCR being overextended is the product of handwaving writing considering that even if the only way to go from the NCR heartlands of the greater LA area to Las Vegas is interstate 15 the route is not really that long even if it has to be done on foot and with pack animals when the NCR is implied to have a working rail network in California itself. The other fronts that they are in like Baja are low level insurgency and teaching tirbals to speak English. All countries go through growing pains but the growing pains of the NCR are exaggerated for narrative effect and to highlight Caesar's propaganda.
