If we're talking about the 4 main expansion packs for NV then yeah I'd say Old World Blues is the worst, it is the least relevant to the actual game and feels more like a side area akin to F3 and F4's expansion packs.
Dead Money is the second lowest for me, while some of the characters are good, it strikes me as something just more of the New Vegas location just with a creepy aesthetic like some Halloween event, it comes up lacking for an identity.
Honest Hearts and Lonesome Road are easily the best, both are distinct from the main game, the former for the more tribal approach to things and the latter for the sheer combat focus. They also expand on the Caesar's Legion by giving us former significant characters in the Legion who are both pretty interesting individuals. I'd argue Lonesome Road is the better of the two but that's honestly entirely because I really like Ulysses character, the whole past behind Hopeville and the final decision being one of the few times in the series I think they really tapped into making the player think about the destruction the Old World caused.
