The movie was about an IRL serial killer. The final scene is the protagonist breaking the fourth wall and staring directly at the serial killer that got away with everything until now. The director knew he'd see the movie and this scene addresses that.
He got away with it in the end anyway. The statue of limitations ran out in 2006.
