I figure Dan and Co. wouldn't have much of an issue until late game aside from Faye. And then It would be interesting to play as a character with declining health; like more sway to your crosshairs, reduced sprinting, generally slower movement, and random coughing fits.
But I'm pretty sure the game takes place over like 3 days, so I doubt anything would really change.
