If I had to guess it's because Edelgard keeps going on about her higher purpose and that's why she needs to create mass scale bloodshed now while THE BOAR just says he's a monster and killing is what monsters do. Dimitri never actually kills an innocent, unless you count invading soldiers as innocents, he just has a major guilt complex over the act of killing itself.

granted its made clear in Yuri's discussions on BL that he never went after anyone that wasn't an imperial soldier, but Dimitri himself is extremely harsh on himself, he doesn't see the women and children soldiers he's slain as pawns of a mad woman that sent them after him, he sees them as people that never should've died and that he's an awful person for killing for any reason.

We also get Yuri saying the only people killed by him were Imperial soldiers. Given THE BOAR is Dimitri's coping mechanism for his hate of killing people in general it's safe to say he's not an entirely reliable source of information regarding the people he killed. He never once goes after people who don't actually deserve his wrath.
