This is standard wisdom in martial arts. But no game ever does it. In games, attacking = vulnerable. Is it possible to make a game where you can block and attack at the same time or would it be too overpowered (or complicated)?

yeah but turtling doesn't allow you to attack though, you still have to decide between attacking or turtling and you become vulnerable when launching an attack giving the opponent an opening to hit you

It's possible and some games do it but most games are too lazy to have a good hit detection system which is needed. Fighting games are especially lazy with their hitboxes.
i don't remember if demons souls or ds 1 had it but i know for sure the later games let you use spears while holding block
In dragons dogma one of the fighters skills is a series of jabs with the shield up allowing you to attack and block at the same time
Every time you attack you are opening up somewhere. Sure you can protect yourself to the best of your ability with proper technique, but you aren't invulnerable.

3D fighters generally have this feature incorporated as sabaki.
Its not a fair technic in 2D fighters because they're generally too fast. You can just throw them out whenever and usually win for no good reason.

Zelda Links Awakening, if Im not mistaken. It could me another game and Im just mixing then on my head, but I remember that as long as the shield is facing the direction of the attack you will block it

it immobolizes you, uses a lot of stamina (more than a regular attack), and doesn't do a lot of damage. basically if whiff you may be fucked
In ds it's very stamina heavy doing a single attack like that, in ds3 pvp you can run around the attack and back stab.
You recover stamina slower when blocking and if they hit your shield you take stamina damage. If you dont manage your stamina and they hit your shield while you attack you drain a lot of stamina really quickly. And also while stabbing and blocking at the same time you are stationary for a pretty long time, which means that if its dodged they can get in and do their own attacks on you as a punish for you missing your attack

seems to me that it would be basically spamming to do the most damage quicker

Off the top of my head Dark Souls does it with shields + rapiers/spears
PSO2 also has a lot of parry-frames on different attack start-ups, and in one instance, even allow you to parry while continuing the attack animation, at any time.
I guess the "realistic" answer is that there is always some opening on the body you can hit, for example in OP's pic his head is well protected but there is a big opening for a body or liver shot under his left arm
I think it would get complicated with a lot of directional attacks and blocks - might require too many different inputs and have uncomfortable controls (it's hard to simulate every body movement with a mouse and keyboard or controller)

Another example would be Monster Hunter, where the Lance move set is (was? haven't played World) based around being defensive while offensive.

You can block while attacking in aLttP, just not using your shield.
Holding your sword out in from of you will block melee attacks and is an active hit box at the same time. The downside is only that your facing is locked.

This. Although its not exactly blocking while attacking, it's pretty much just that.
There's also some attacks in tekken that have a parry window, so if you try to attack during that window before the move comes out you get counterhit.

Dark Souls, Dragon's Dogma, and Monster Hunter all allow it in some capacity.

Tons of games feature "super/hyper armor" that resists or cancels stagger and sometimes damage during attacks, or straight up counterattacks where you receive an attack, nullify it, and then deliver an attack that itself cannot be defended or countered.

Boxed for 3 years, and competed. Absolutely worth it, join a good gym and get sparring. Make sure the gym you're at isn't full of guys who have something to prove and you'll have a great time.
Quality of gym you can honest to god tell by ethnicity. Lots of hispanic/black people it's probably a good but a little rough, you probably want a good mix of soft looking white people with hispanic/black people so you can work your way around.
Even the person in that picture is vulnerable at the moment of attack. The jab may be the safest punch, but it's still open to be countered if timed correctly.
And then you equip the caestus and swing your arms around while leaning your exposed head forward, just asking to have your jaw broken.
I did a boxing class from mid January to end of Feb. Was hella fun. Got more fit, started to see some real muscle definition and learned some basics of fistfighting.

UFC games, they allow you to get away with stuff like blocking while attacking and dodging while attacking because attacks are tied to a stamina meter and lower stamina makes you easier to knock out

Tekken has moves that duck high attacks and dodge low attacks, stances that automatically parry fist attacks or low attacks, etc.
UFC3 has pretty awesome striking. I was impressed.
