I had this idea that was sort of like Demonophobia. Here it is, copy-pasted from the times I've mentioned it, per usual.

You play as some poor sap who wakes up in a weird complex that's been subsumed into a hellish environment. I don't mean Doom-esque demons, I mean shit that looks like it belongs in a Silent Hill game. However, things take a drastic turn for the unexpected when the protagonist starts fighting back. The horror lies in a few key aspects of the game

>The combat is fast paced, and not in the "Movement speed" way, I mean that the claustrophobic environments mean that danger is always imminent during any combat encounter. You can’t just run away every time
>Consider stealth or combat
>The injuries the protagonist takes start stacking up, to the point where they start to look less and less human as the game goes on. They don’t exactly mutate, but they just get injured in such a way that they look inhuman. Same with the supplies they pick up
>The degradation of the protagonists mental state. They start out scared, alone, and fighting for their life, but by the end of the game, they've basically become a nearly silent killing machine that mauls everyone and everything that gets in their way of escape. Potentially even other human survivors.
>The violence is really unsettling. Blood doesn't spray everywhere in a cartoonish fashion, but kind of just oozes from the wounds of enemies.
>Horrific guro death sequences

this. if you reduced mhw monsters even with their current appearance. but made them 3 shot you and cant heal in combat they could be scary... except maybe except kulu yaku. just tack on a gory violent death when they win and reduce their health to like 15 hits forcing a retreat and death on the third round. make them ambush and better camouflage when not movingand bam theres your horror game bro. just make the story some survive the island and war the rest youve got a replayable fun scary horror game. remember even a mountain lion is scary if its in real life. but that doesnt make you helpless if youve got a large knife.
