Second playthrough. Same as it was for the first Xenoblade, oddly. For whatever reason these games never click with me much outside of enjoying the gameplay on the first run, then go from 8/10 to 10/10s the second time around.

Yeah, but what was considered 'US' in XB2's time is muddy. The Siren info says the orbital ring was set up by a unified government, but the signs inside are all in English and have references to the US specifically, so for all we know it's some weird timeline where the US constitutes a much bigger part of the world or some shit.

Never, I was waiting for it to happen because it happened several times in the first two games, but 2 just doesn't like up to 1 and X - to 1 in terms of story and to X in terms of gameplay and world.

You mean the world tree? Yeah, you're almost done. You can just beeline through most of it, it's pretty straightforward and the enemies are by and large, easy to avoid. Even the big mech one in the last big room.
Does the combat in this game ever get good? It just feels like "do the same combo over and over" and other than that it's just a stat check to see whether or not you win.

Malos as playable main character when?
If you're at the point where you're regularly using the full break/topple/launch/smash driver combo in battles and mixing it with the specials, then yes, it really doesn't get much more complicated than that outside of a few boss fights with gimmicks. If you're just in chapter 1 or 2 (or the early part of 3) then it does get more complex later on.
