It's a heuristic. If you had precise, machine-level measurements and a big data set of correlations, you would be able to tell all sorts of things about a person just from something like cranial measurements, to a very high degree of reliability. You can do much of this without machines, just by looking at people, but this is obviously not as accurate and is more prone to error. But often still correct.

The reasons for this are fairly simple biologically; healthy and 'fit' organisms of all sorts (not just humans and not just creatures with faces) tend to be symmetrical with respect to any sort of 'doubling' feature. So a 'fit' centipede will be more symmetrical along the axis of its body and will have more symmetrical legs than an unfit centipede. The fit centipede will be proportioned correctly, while unfit centipedes will have body compartments which tend to veer from the median proportions of all centipedes.

Humans are no different; you can immediately pick up on a lot of different genetic malformations simply by glancing at a person's head. The brain is part of your genetically-expressed biology, so malformed faces will often indicate malformed psychology.

not exactly. profit in poker is the sum of mistakes your opponents make against you minus the mistakes you make against them. each hand takes about 3-5 minutes to play and you only need to see about 10 hands or so from someone to tell if they're making mistakes at a higher or lower frequency than the general player pool. so even if you do look like an idiot good players will recognize that you know what's up pretty quickly based off of how you're actually playing and bad players don't think about these things at all.

To my knowledge, the theory is not that all maniacal individuals display the phenomenon, but that the phenomenon is more likely to indicate a maniacal individual, just like not every dark skinned person is violent, but disproportionately, etc.

how it works in America is there is a board with a list of players waiting in line if all the tables are full. if all the tables aren't full, you can sit wherever there is an open seat. once you're seated you can ask the floor manager for a table change. if you tip the floor manager a $5 here and there regularly thought they'll just let you sit wherever and whenever you want as soon as a seat is available.

Fair enough. I just don't rely on it as an indicator of anything. But relaxing around anyone is a mistake anyway.
