I don't know about his characters being androgynous, in terms of physique. His male protagonists have way broader shoulders and tend to stand nearly a head taller than his female characters. And while they do tend to be slender, it kinda makes sense, they're usually empowered by some unnatural source, be it Mako energy, Guardian Forces, or being a fucking solid ghost, that grants them some degree of innate superstrength.
Big muscles come from muscular hypertrophy, you have to bulk up by lifting progressively heavier weights that overload your muscles and cause them to degrade and then rebuild with more muscle fibers. If your baseline strength is enough to let you casually swing around Cloud's Buster Sword or Auron's Katana, then it's gonna be really hard to find any object you can lift with both the weight needed to induce hypertrophy and the structural integrity to survive you lifting it.
It makes sense his characters should be slim and toned, like Spiderman, instead of massive and musclebound, like Bane.

Dude, that's just cultural. Anime and JRPG characters have weird hair, it started to distinguish characters apart and became a genre staple. Once they let Nomura take the reigns of character design from Yoshitaka Amano and he largely stopped giving characters blue, green, or purple hair, limiting them to a color palette human beings can actually have, he had to make the hairsyles distinct instead.
And by the way, very few of his characters where "dresses". They wear badass longcoats, like the Punisher or Hitman or John Constantine. If a dude's in a dress in his games, it's usually a villain and you're gonna fucking kill him eventually, so no worries.

No, it's not. Literally in the games themselves you can see the difference between the visuel keiesque protagonist and the average characters. If you do anime hair, you stylize your whole character in an anime style. SE doesn't do that. And if other Japanese studios do a more realistic settings, they usually don't check off all the tranny boxes on the list. Pic related for a positive example.
