NK's biggest trade partner, or rather, only trade partner is China. China has a vested interest in keeping NK in a stable enough position to keep Korea divided, because the worst case scenario is NK collapses internally, SK moves in to create a unified Korea, and then China suddenly has US troops sitting right on its border.

So you can pretty much say China runs NK. Kimmy has shown he is willing to defy his overlords in Beijing, what with all the missile tests since last year along with aggressive nuclear weapons research likely more so to gain a diplomatic edge against the Chinese than the US to safeguard Kimmy's position. China has shown it's against this, and has cut many diplomatic ties to NK ever since Kimmy's decisions to go against Beijing's wishes for a neutral and passive NK.

It's probably not out of the question to say that China can easily sway many inside the NK's government to their side with the promise of stability. No one would want to abandon their safe and wealthy life just because some kid with a death wish wants to defy the Chinese and runs the very real risk of causing internal collapse.

So long story short, the Chinese run NK. NK might as well be considered a Chinese puppet state, because without China it'd likely fall apart as a state within months once the military starts to go hungry.
