What are cave systems during WWII island hopping, tunnel rats in Vietnam, GWOT and caves in AStan/Iraq/Syria, tunnel/sewer/subway systems under cities in an increasingly urbanized world, Best Koreans as mentioned above, etc.
Challenges - it's an extremely restrictive CQB environment where there may be environmental hazards, specialized breaching required, plus difficulty of maintaining supply when you're a mile down some sewage tunnel that's being cleared. Obvious need for specialized training.
