It actually means that you were so good at medicine, that your university was willing to voluntarily let you spend an extra 3-4 years mastering an extremely difficult and highly specialized area of medicine that most doctors aren't qualified to practice.

It's sort of like how when you finished high school, you went to a work training program for niggers and were put on food stamps, while everyone else was working on their SATs and getting into college. It's not that you were so smart that you "finished" school before they did. It's that your teachers, parents, friends, etc. realized that you were too lazy and stupid to succeed in life at anything other than flipping burgers and collecting welfare. Your classmates who got into university or went to trade school were smarter and more productive than you, so they were given the opportunity to pursue actual careers. To complete the analogy (because you a probably too stupid to piece things together yourself), your classmates who got to go on to college, trade school, etc. are like the doctors who go on to practice as specialty physicians: they were the smart ones (e.g. Rand Paul).

Depends, winning a libel case isnt easy and requires the accuser to establish that what was published (yes anything on twitter is considered published) was untrue, that the person who published either knew it was untrue or through great negligence failed to realize it was untrue, and that what was published caused financial, social, or physical harm to the accuser.
