The US healthcare system operates at near capacity in a normal year. A highly infectious disease with a 1%, give or take, mortality rate will overwhelm it rapidly if it isn't quickly prepared for. It'll collapse the system. You don't need bodies in the street and plague carts collecting the daily dead, you just need to spend a month or two slightly overwhelming the hospital system each day, day after day. When the hospital system collapses, you have monumental disruption, whether or not it's the fucking black plague.
