The biggest one of course is the age
Then you have several factors, way of living, pre existing medical conditions etc
If you have high blood pressure, diabetes, chronic bronchitis, cancer, overweight, etc the chance of death is extremely high
The chance of a young person, without medical conditions dying is very very small
The people who die, are very weak or very ill anyways
The normal flu could kill them, but what is special about this virus is simply that it really puts a strain on your respiratory system. Which means, that many people quickly need medical help. When you are sick, and you have a problem with getting oxygen, your organs cant work proberly, and your chance of dying is increased
Now imagine someone being overweight, and having lets say high blood pressure, now not being able to get oxygen into his body, because the lung cant fullfil this task anymore. Its critical.
Most people are sick, but no one talks about it. Most people nowdays have severe problems in terms of health, but can live somewhat normal trough medication.
Those people are a risk group.
Then the thing with infections and deaths. Italy has a culture of being open, speaking loud, touching, meeting with grandparents on the weekend, being outside, strolling along the streets, sitting in caffees = extreme spread of the virus. Italys people are very old, and the health system is overloaded, meaning many old people get infected, and die
