Why is it legal for hospitals to bill insurance companies way more than what they would charge a cash paying customer?

I see this shit all the time, I used to work for a medical software company and I'd see hospitals charging up to 10 times more on an insurance bill than what they'd charge for a cash customer.

It's fraud and it's one of the biggest reasons shit has got this expensive in the first place.

Because it doesn't matter what we bill. The insurance has a set payment rate for each billing code and will only pay that much. We bill for the inflated AWP just to make sure we don't undercharge if the insurance is willing to pay more. In practice the actual reimbursement given to us by the insurance is usually lower than the cash price we would charge a patient.
Do you honestly believe that insurance can afford to pay 10x more than you would out of pocket just because you pay a $30 premium every month and a copay on your meds? The insurance company would go bankrupt if that were the case, instead they pay their CEOs billions in bonuses every year because they have the negotiating power to pay much less than you do for those services but still charge you much more than you would be paying cash in premiums and pocket the difference.
