i like the way he isn't ennobled by suffering. instead of rising up and overcoming his deprived origins he just ends up emotionally damaged and bitter and paranoid and pathologically self-destructive, unable to adapt once he's out of the survival mode extreme poverty life. it's like a reverse underdog story, instead of taking the bad hand he's dealt by life and rising above it he takes a bad situation and succumbs to it, fucking his own life up even more at every turn.

there's a story about Chinese doctors who came to Japan in search of elixir of life and never went back (they didn't want to be executed). very likely that the doctor who made the medicine was one of them or their disciples.
Muzan got the medicine from a doctor. Who's to say that doctor didn't get it from China which could travel down the Silk Road to the MidEast and Europe?
