The old guy they have been chasing is definitely a stand, which makes all the times Rai questioned if he was really 89 make a lot more sense. He's supposedly an old man that needs a cane to walk but he consistently out runs the other characters. We still don't know if the "head doctor" that gave the conference/presentation is a real person or not though. Its possible he exists as a person and his or someone else's stand is copying his appearance.

someone ever noticed the HD is always seen through frames(windows/door/screens/)?
tooru is the stand user and is protecting the HD
the first time the gang chased the HD in the hospital and that unkown stand tried to ambush gappy, thats when the calamity thing activated
after that, everytime someone sees the HD its just an illusion/refelction of him casted by the stand
starting with the glass gates in the hospital, the back window of the bus, when mitsuba saw him through the window etc. Its always some kind of frame
the last chapter especially made it very clear thats how this stand moves
he was real on the stage scene in the hopsital and when yasuho got flushed down by jobin, thats why we could see his face

>The stand at the hospital took the form of the HD after he touched Gappy
>It cannot be seen by people that don't want to pursue the HD
>The HD that the gang chased before bumping into Toruu was either the real one or Toruu in disguise
>The one after that is the stand
>That stand had the same designs as Toruu, we are assuming that this is Toruu's stand
>The HD is a real person, he is definetly a rock human and is also probably Toruu's boss, Toruu acting as a body guard of sorts, his ability fits this role well, as both a distraction for pursuers but also as a means of killing them
>The stand can appear in photos, whether this is as a result of his powers or just Araki retconning how stands work remains to be seen
>The stand works on a Karmic level, inflicting worse punishment if you have a lot of bad karma, the guy in the prison killed kids, so the food in front of him started to rot just because he was there, and died in mere seconds afterwards

I do agree with Tooru being the user, he acted super suspicious in the introduction of the stand and it mysteriously appeared when josuke called him out on it. Tooru also only seems to care about Yasuho when she mentioned "healing" so I assume he knows about the Rokakaka.
I hadn't considered it before now but what if Tooru is actually the head doctor? Its possible the old guy we thought was the head doctor is only a stand and was never a real person. The old guy definitely moves too fast for a person that needs a damn cane to walk, he basically teleports in every different panel he's in.

Come to think of it, Yasuho never actually got karmic punishment when she saw doctor's back at because she never kill anyone. Mitsuba killed doctor wu, Tsurugi killed doobie wah user, and Josuke killed a lot rock human.

Toruu being the HD doesn't make sense, he is an old guy, he worked at that hospital for some time, don't you think people would notice that?
Plus, you can't just create a face out of nowhere, this stand used the referrence of the real one, the one that can talk and has a face

Assuming Tooru is the head doctor he would be working with the other three rock humans that were pretty high up in the hospital, he could probably get away with it. From what I remember not a lot of people ever actually went to see the old guy. Its possible the stand just took the form of a random old dude he passed on the street or something. The hint that Tooru might know about the Rokakaka in the new chapter is what has me thinking about this. He only seems to care about Yasuho being hurt when she mentions "healing".
