Yeah it's a little unclear if hes talking about himself or tanjiro. Half the translations are one way or the other.
Doesn't Muzan have like twelve hearts? So it should have been plural if he was talking about his (also not sure why his hearts would stop beating early before the sun got to them) so it really makes me think hes talking about tanjiro.
Especially when he says that "if you can escape the swift death" i.e. hes dying and hes not breathing and his heart is stopped.

Tanjirou was probably already a warm body when he was inside Muzan, hence the "cells are not destroyed yet". In the page it looked like Muzan was inspecting Tanjirou, so most likely the "heart and breath" refererred to Tanjirou.
