Casca going crazy was a necessary device for Miura to get himself out of the corner he had written himself into at the beginning of the story. He's confirmed in at least one interview that when he first started writing Berserk he did not have the story planned out yet, by the time Guts first confronts Griffith in the Black Swordsman arc he still had yet to come up with the reason why they hated each other so intensely.

Fast forward to the end of the golden age arc and suddenly Miura had a lot loose ends from the beginning of the story that needed to be connected with the present. Some of these he did well, (Guts's going off on his own, no longer letting anyone get close to him) others not so much. (Where was Casca during the Black Swordsman arc?)

I mean more of an explanation of why she wasn't with Guts. Casca as she was presented in the Golden Age arc almost certainly would've accompanied Guts on his revenge quest, even Guts himself protested. Miura needed to explain that and her insanity was basically a cheap way of doing so.

I think it’s just the way Miura presented the rape scene. Yes she bled, but after she was really wet and ahegaoed the whole time. This is also her second time so take that as you will. Also the way she moaned Griffith’s name was incredibly suspicious, and only served to make the whole rape scene look more dodgy. At the end when she loses her mind it’s more like ‘you fucked me so well it literally made me retarded!’. Plus all of her flashbacks tend to show Wyald’s and nameless Apostles’ almost rape. It’s as if the actual Eclipse as a whole was what traumatised her.
Yeah, her reaction during the Band of the Hawk reunion was fucked too.
Yeah it really seemed like she was settling with Guts. On another note, I don’t think Femto raped Casca to spite Guts because his status as a member of the Godhand puts him above that. He planned on reincarnating in the real world so he needed an anchor to the physical world, AKA demon child. Guts and Casca were clearly meant to be spared
