I personally think Deku wasn't born quirkless. He was actually the original owner of All for One's Forced Quirk Activation quirk. The good doctor lied about the toe joint and the x-ray was just some random kid's. All for One stole the quirk from him.
Ever notice how personalities and quirks almost always match up? Think about Deku's quirk analysis obsession. Obviously having a quirk that allows you to use other quirks would naturally come with the ability to figure out how other people's quirks work.
The reason he can use the other quirks stored in One for All is because he still has a body that is capable of activating other quirks.
