The difficulty with Rei is that whatever you get her is inconsequential because she deserves more. One should strive to get her that which is invaluable and that which tantalizes and lulls the soul rather than the eyes. Also, I've always hated how everyone treats her nice only when she is needed so I would try and remedy this while combining it with the former.
Obviously, despite the fact that I would want the entire world to know it's her birthday and to celebrate accordingly, I think the form of celebration she would appreciate the most is a meal or cake cooked/baked by you with diligence, care, and effort (these last three elements are what she would appreciate, not so much the cake). Get her a few books that have impacted you in a positive way in hopes that it'll provide the same joy to her and finish it off with a simple happy birthday card that denotes how happy you are to be able to share parts of your life with her.
