He mistook the copilot copying ATC clearance for takeoff clearance and it was some hillbilly airport with one runway, they were all using nonstandard lingo. That said, the blame is still entirely on him for being an impatient jerk. He probably tailgated people on the highway too.

it was only one runway for the whole airport and the 747s were asked to "backtrack" - taxi down the runway, then turn around and take off - because the one taxiway was jammed full of other delayed aircraft

There was only one runway and one taxiway. The 747s were forced to use the runway for taxiing because of congestion near the terminal. Fog meant that the KLM couldn't see the Pan Am still taxiing on the runway, and nonstandard communication between the KLM and tower meant that Van Zanten thought he had clearance to take off when the Pan Am was still on the runway. Pan Am tried to turn out of the way but the KLM was going too fast and they crashed.
