Ross. His insecurity over her job and her hot coworker caused the fight. He slept with the copy girl, which he had every right to do, but then he tried to cover it up, and would have kept it secret if Gunther hadn't spilled the beans.
Rachel, however, is at fault for literally everything else after that throughout the series. Ruining his relationship with Phoebe's friend, trying to crash his wedding, keeping the baby a secret, etc.

Also, Ross didn't actually think they were on a break. He thought they were broken up. He tells the guys "once one of you wants to break up, you break up." They convince him to call her, which Rachel was desperately hoping he'd do, and they immediately start to make up and regret the fight, until Ross hears the coworker in the background. He hangs up, gets drunk, and copy girl is all over him.
So Rachel fucked up in letting coworker come over, but she did reject his kiss immediately and made it clear she still wanted Ross. Ross's suspicions about the coworker were justified, but that's not the issue, as Rachel was upset she didn't trust HER. And again, he was going to keep the tryst with copy girl a secret after the fact, which led to Rachel breaking it off for good because she simply couldn't trust him anymore.
All the "we were on a break" stuff came later as a recurring gag, because Rachel accused him of cheating on her (not true unless you consider a formal verbal break up necessary,) and Ross would counter that they were on a break (also not true as neither actually thought this at the time.)

No, but keep in mind that Rachel wasn't going to the movies with this guy. He was her coworker, and despite working late nights together, Ross had to trust they were working. Rachel was naive that the guy wasn't interested in her, but that doesn't change the fact that she wanted Ross to trust her.
Remember, they had already had this fight in an earlier episode, and Ross justified his insecurity due to his ex-wife coming out as a lesbian and leaving him. Which is understandable, but shouldn't force Rachel to avoid all men Ross might be suspicious of.
Also remember, the break up fight started because Rachel was working during their one year anniversary, and Ross was upset about it because she's always working. So he showed up to the office with a picnic that was extremely disruptive and she angrily sent him away. Ross argued it's just a job, but Rachel - a spoiled princess who had previously been cut off and forced to serve coffee - was finally doing something with her life that made her feel worthwhile. Ross was fundamentally not understanding her position, which is why I say he was ultimately at fault for their break up.
But again, Rachel was at fault for everything else, because she ended up extremely jealous of all of his subsequent relationships, yet never came out and said she wanted him.
