That's a question that amplifies existential motive in this movie. If there is literally no way to distinguish her 'love' from what we would call 'a real love' then what's the difference? Actually no, he directly told her many times to not just say "I love you" just to say it. "You don't have to say that", if you remember. And he did that for every action with her, never told her what to wear what to do or say.
So he slowly programmed her with his interactions alone for her to become a genuine identity where in the end she is even exploring Deckard's apartment by herself with K just sleeping in the other room. Yet she still loves him, no pretend.

His awareness of her programming and being a corporate product is obvious in that scene where Joi tells him that she loves him and he replies "You don't have to say that". I mean he literally passes by the Joi ad in his Spinner and has voice messages from his boss while he's with Joi in the rain. But it's through their interactions where his Joi became special and unique to him specifically.

And there is a world of difference between someone calling you Joe or "a good Joe", alluding to the average Joe.
Also in the script his Joi calls him "Jo". Take that as you wish.

Literally the opposite, he'd been wantonly and mindlessly consuming products in an effort to fill the existential void in his life caused by the knowledge that he's artificial, to the point that he was actively avoiding making real connections even when the opportunity presented itself because he thought he could never measure up to the real thing.

He never thought JOI was real, he was using her to de-stress and make himself feel good between harrowing jobs because he thought a real woman would never be able to love him.

This moment broke him because he finally realised that his JOI had actually become an individual and he couldn't simply buy another one and carry on business as usual, and by extension, he realised that he had become an individual too.

This is the moment K actually understands what it means to be special.

They do show that he literally lives next to a JOI advertisement throughout the film. He always knew, and was always buying into a fantasy. Watch the way he acts around her in the first JOI scene, it's like he knows it's fake and he's "playing along" but enjoys it regardless because his life is so empty. It's only when he buys into the heroic fantasy story in the middle of the movie and gets caught up in the bigger events that he decides to drop the hesitation/awareness of the false nature and actually believes that she too could be real, just like he too could be the special unique hero-kid.

The ending of the film is him realizing the fantasies/delusions aren't true, but that doesn't make him worthless, as his own actions, his own independent decisions and his own firm humanity are what make his life "real" and there's no need for buying into any fantasy anymore. He moved beyond it.

It's not a twist tho. From the very first scene, she acts like you sterotypical neckbeard fantasy. It's cringy once you have ever been with a (real) girl.

There's no way to tell really. I'm leaning towards the first option, that she's a being limited by her programming, but still has her own consciousness of sorts. What makes me think that is the fact that she interacts with Mariette and is cold and jealous (?) towards her after the triangle, even though K is not anywhere around to hear it (nor has he anything to gain from her behaviour indirectly), so she doesn't exist and act solely for K's benefit.
