OP here, thought there was some clue I must have missed, since I watched it on the plane. Did a quick duckduck and the best answer was no, he really is his father, the whole thing got suppressed and she was sectioned and heavily medicated to hide the truth, which is what I was inclined to believe, due to that one slightly clinching it in the sadness stakes.
Main arguments for the theory are
1) the reaction of Thomas Wayne when Arthut confronts him in the toilets, he says more than necessary, he doesn't just say, "nope, not your father" but gives him a whole story of her delusion and adoption straight away, which is typical behaviour for a liar.
2) how would a delusional single woman be able to adopt him in the first place, especially if she had been in arkham asylum for a serious while

It wasn't an unreliable narrator movie apart from the black girl. The fact that the more important "mystery" of his parentage was left a mystery because of inconsistent and incomplete on-screen evidence is simply frustrating because of the lapse in reliability opened up by the black girl romance issue. So it's only a coinflip of who you trust more to be lying scumbags - joker's mother or thomas wayne. Other than that it's a question of which story is more dramatically appealing to you, but even though there's no factual confirmation one way or the other, the tone of the relevant scenes kind of suggests to me they intended joker's mother to be the liar, which isn't what I would have wanted to be the case logically, but the film is shit if you watch it with Wayne as the bad guy.

Her delusions about Thomas after he was seemingly kind to her mirror Arthur's own with his neighbour. It's why Arthur's romantic delusions come crashing down in the scene after he discovers the truth about being adopted and his mom's delusions and why he murders his mom afterwards - it's all one interconnected string in his breakdown.

It's the same reason I think he killed his neighbour, it fits with it too well. He angrily rejects having his fantasy torn away by killing the ones he blames because just like his mom he cannot accept he's at fault on any level.

The adoption records are there, the behaviour from the mother in the movie shows signs of mental derangement, and the reveal that he's not Wayne's son is a key point in Arthur's development. Throughout that part of the film Fleck is being built up, as far as a first time viewer is concerned, to be a successful comedian, in a growing relationship, with a noble cause to prove Thomas Wayne's paternity. The reveal of the truth hits the audience and the character, providing real weight for the rapid downward spiral. Wayne being the real father would kind of ruin that whole stage in the character's development as his sympathetic noble cause would've been valid after all.

I disagree thematically it makes perfect sense that he would be Wayne's son, but that he would not believe that he was after the fact. Think about it really, even if they aren't related the Joker created the conditions possible for batman to exist, they are foils to one another and thus, perfect logical ends to infer that they could be related.
