They've been back and forth in the canon. Much of Joker and Harley keeps getting retconned, but there were multiple times she was depicted as a competent doctor but with an underlying instability Joker used. The gritty, "realistic" graphic novels in particular tend to portray a far shrewder Harley and Joker. Either way, I think it's a more interesting interpretation and adds more nuance than "slutty doctor bangs criminal"
