I seriously don't know if some of you are trolling or retarded. It's not that he knew for sure, it's just that the coincidence is so strong he was able to connect all the dots in his head. He never suspected Walter of anything ever (ok, maybe in the beggining, I don't remember), he trusted him. That book made him think what if it was him and damn, he was a chemist teacher, he acted in weird ways before several times, it all clicked at once. Honestly, it was a perfect scene, I don't like the last season but this was done right.

You have the initials of both of the important players: WW and GB, and the book is located in Walt's house. Hank spent time earlier in the show trying to figure out who the mysterious "WW" that Gale mentioned was, before Walt steered him toward Walt Whitman. The inscription reading "my OTHER favorite" would clue Hank in that it wasn't Walt Whitman, but rather someone else. It could only be Walt at that point.
Besides that, Hank knew the case better than just about anyone else, but he was blind to Walt because he was family. He only needed a hint like this to point him in the right direction. It's not like he needed the book to spell out the entire case to him.
