I hadn't though about it until now, but apparently they were feeding him enough Iroh for him to get built like this.

Iroh was probably in really good shape when he was the dragon of the west so it's easier for him to get into shape and irl prisoners can get in really good shape despite having a bare minimum diet so it's not impossible to get like that on oatmeal and a loaf of bread

Iroh got buff through calisthenics and sheer willpower. If they were still feeding the man three square meals with pork and vegetables, sure, but if he was a regular ancient prisoner the man should have been too weak to stand.
