I like how the one Boomhauer's perspective episode implied that he's the only one with an intelligible accent and that everybody else has the ridiculous unintelligble Texas hillbilly accent.

Absolutely correct. As someome who takes calls from people in the South, I've got quite a few Boomhauers and all you really need to suss out are nouns and a few context verbs.

According to Mike Judge, Boomhauer's rapid speech fire speech and accent came from a voicemail a fan left for him after he was done working on Beavis and Butthead.
The message he left was done exactly the same way Boomhauer's speaks, he thought it was the funniest thing in the world so he decided to base a character off of it.
As others have pointed out, the joke is that he's hard to discern or understand fully, but typically comes off as sage, insightful or a the voice of reason even though he's typically easygoing and tends to go with the flow of things.

Third best boomhauer joke was the flashback where they were playing football in high school, in the red zone, just before the hike. He then sees an opening and just flat tells his team a new play, telling who to do what and what they’re going to do. the other team cant understand his directions, while his team all understand perfectly, giving them the touchdown
