If you played Silent Hill then you should now by now that the main character Heather a 17 year old girl is pregnant with the God of the city which turns out to be "Lilith"
Heather is part of Alessa, the original girl meant to give birth to the town god. The plotwist was that the town mistook Alessa for a vessel for god and not the god herself, this cause a severe torture session in which a cauldron exploded and severely burnt Alessa, making her live the rest of her days in the hospital and sedated by a drug that gave her twisted nightmares that later materialized into the town, consuming it. The town's cult, a doomsday christian sect, seeking to control Alessa, tries to perform another rite, but Alessa fools them and splits herself, gives birth to
Cheryl which is no other than Heather.
The monsters of Silent Hill, obey the god, seeking out punishment to those who are sinners while leaving the innocent apart, this is why sometimes children can be seen playing and then disappear into the mist, once again returning to the real world to continue their daily lives in Silent Hill.
Currently, the cult is trapped in purgatory for what has done and its creatures torture maul and hunt the cultists and their children children till justice is repaired.

I played SH3 as recently as two years ago, but I have to admit I never paid much attention to the lore. I got suckered in by SH2's completely symbolic story, and then these more literal plots about cults from SH1 and SH3 just didn't seem that interesting. Frankly I thought the most interesting thing about SH3 was the "they look like monsters to you?" line. I like the idea of Heather just stomping around massacring innocent townsfolk because she's nutso. That also makes it a lot more meaningful when the designs of the monsters reflect the main character's emotional state like how the nurses reference James's sexual frustration and Mary's hospitalization, or how the Closers in SH3 look like big clumsy girls. SH being a real town with a real cult seems totally uninteresting by comparison, same way Endless in Twelve Forever is much more interesting (to me) as a reflection of Reggie's internal emotional state than as a real place.

The first Silent Hill for me had the best atmosphere when playing as a child. I would always play in the dark for immersion where no such things as jump scare but the ambient horror to keep you guessing. Ah, yes those were the good old nights!
