My admittedly somewhat edgy headcanon is schizophrenia. Disorganized behavior, disorganized speech, disorganized thought, odd affect and, oh yeah, _she regularly hallucinates getting whisked away to a magical island_. She also injures herself in the final episode and then blames it on the sixty foot tall woman she regularly sees, the one she thinks is following her around and causing her body to change. Todd and Esther are just kids playing, they don't know how real this all is in Reggie's eyes. When Todd gets angry and says that Forever "isn't real," she objects. It's completely real to her and not just a game.
It also always seemed odd to me that Ogden was so surprised to experience Forever in Babysitting Forever. He's a kid, shouldn't he understand he's in another kid's imagination? Reggie is an older kid, and he calmed down when she played a more age-appropriate game with him, but then Todd and Esther were also surprised the first time they went there, and they're the same age. The unique thing about Forever as a realization of Reggie's imagination is that it's literally real to her while Todd and Esther and even her teacher think she's just being creative. She's a Louis Wain.

Here's my take on a rebuild twelve forever. Todd started third impact trying to get Gwen back but failed and went in a 14-year coma. After that, he wake's up to see one of he's younger sisters now older, escort him to the SS Dracula. When inside, he sees a pissed off Reggie.
